Website prior to receiving the H1N1. Please follow

so:
1. Go to https://h1n1vaccine.odh.ohio.gov
2. Look for this: New Individual / Family to Pre-Register Click Here
3. Click on the link
4. Register*
5. Print off pre-registration paper work when you get to the screen that assigns a patient number.
6. Bring the paper work with you when you receive the vaccine.
